PrEP on Trial: the Future of HIV in Indonesian Policy Worlds

In 2012, the first pre-exposure prophylaxis (PrEP) drugs, billed as a pill a day to prevent HIV, were authorized for use in the United States. Heralded as a transformative prevention technology for gay men and trans women in particular, one that encouraged new forms of self-management and risk mitigation practices alongside condoms, testing, and treatment, PrEP has since been incorporated into the global HIV prevention toolkit. In reports, policy documents, and community organizations, PrEP is uniformly described as necessary to accelerate the HIV control response and meet the global target of the “end of AIDS” by 2030. In line with this dominant policy narrative, governments reliant on international donor funding for HIV programs are now encouraged to incorporate PrEP into HIV programs for MSM, transgender women and other “key populations” assessed as meeting a specific risk profile. This is the case for Indonesia, which formally approved PrEP for a trial in 2021 (United States Agency for International Development (USAID) 2021). Although initially announced in 2019 with a considerable degree of community support, Indonesia’s PrEP trial was postponed both due to the COVID-19 pandemic and subsequent bureaucratic delays. Nevertheless, with significant pressure from international donors and support from the Global Fund, USAID, and Australia’s Department of Foreign Affairs and Trade, a PrEP trial commenced in April 2022 across seven provinces in Indonesia. Key populations who agreed to undertake an array of tests and routine clinical monitoring, would obtain access to a 30-day supply of a single pill combining tenofovir disoproxil fumarate (TDF) and emtricitabine (FTC) – generic Truvada – from one of 34 primary health clinics at no cost. As it circulates in policy, clinical, and community spaces, PrEP is transforming the temporal horizon for HIV in Indonesia and other postcolonial settings where access to healthcare remains thwarted by entrenched global inequalities. The Surveillance Cyborg

Editor’s Note: This post is part of our ongoing series, “Queering Surveillance.”  Surveillance is an embodied experience, both being watched and watching. The sheer number of concert-goers recording Cher’s “Here We Go Again” concert this past year with their phones had them trade singing and dancing for an act of documentation. Whether the recordings are to remember the experience later, share the experience with others, or to simply document one’s presence in that space and at that time, recording the concert on one’s phone becomes an experience in its own right. They are present in the space, but their attention is about both what is happening in the here and now and the recording that filters the experience in the future. Their phones and recordings are central to their embodied experience, fused into one like a cyborg traveling across space and time in the moment. Add to this that countless concert-goers are recording the same concert from their individuated perspective, and thus the concert becomes infinite and virtual—of course, the way Cher was always meant it to be. Towards a Queer Art of Surveillance in South Korea

When is a face not a face? With the launch of the iPhone X that boasts facial recognition capabilities, the individual markers of one’s face tie one’s identity to the security of their phone. Yet it also makes the face complicit in forms of self-surveillance, as it requires definitive facial proof to access one’s phone. It produces the face as evidence of one’s identity that supposedly cannot be forged. In this instance, one continuously uses one’s phone to surveil one’s own identity—with the face becoming a safeguard against potential security breaches. Small-scale, yes, but surveillance need not always be connected to sprawling security apparatuses and institutions. So we ask again: when is a face not a face? When it is used to distinguish a body as a body rather than as an individuated person? With this post, we seek to explore possible answers to this question in the context of South Korea, by focusing on the role of self-surveillance in the politics of queer student activist organizations. Emilia Sanabria on Bodily Plasticity in Brazil

Each year, Platypus invites the recipients of the annual Forsythe Prize to reflect on their award-winning work. This week’s post is from 2017’s honorable mention Emilia Sanabria, for her book Plastic Bodies (Duke, 2016). In early November of last year, Judith Butler was attacked during her visit to Brazil. She was in Brazil as a member of the organizing committee of a conference on “The Ends of Democracy,” held in São Paulo. Her visit drew protests from the Catholic extreme-right who assumed she was in Brazil to speak about gender and undercut catholic family values by questioning the biological “facts” of sex.
